Diagnosis of rare brain disorder may take months

NEW YORK (Reuters Health) - Doctors often initially misdiagnose people with Jakob-Creutzfeldt disease, a rare brain disorder caused by misfolded, infectious prion proteins, according to new research.
Jakob-Creutzfeldt disease, or CJD, is marked by rapid brain wasting and dementia and typically kills patients within a year after symptoms appear. According to the National Institutes of Health, there are about 200 cases each year in the United States.
In the new study, researchers found 97 people with CJD received an average of almost four different diagnoses each before doctors made the correct call. Those alternate diagnoses included autoimmune disorders, Alzheimer's and depression.
Researchers said the early symptoms of CJD can vary widely, depending on where in the brain infected proteins accumulate.
"There's a lot of other things it could be, literally any disease that could affect the brain in a relatively rapid way," said Dr. Richard Caselli, a neurologist from the Mayo Clinic Arizona in Scottsdale, who wrote a commentary accompanying the study in the Archives of Neurology.
He said the search for a correct diagnosis, with multiple failed attempts along the way, is "obviously a very traumatic thing for families to endure." It can also be expensive.
CJD is not treatable, so although catching it early prevents patients from having to undergo more tests, it won't change the course of the disease.
On the other hand, Caselli said a number of other conditions that may come with similar symptoms, such as infectious meningitis, can be treated if they're diagnosed quickly. That's why neurologists try to exhaust every possible curable option before landing on a diagnosis of CJD.
Dr. Michael Geschwind of the University of California, San Francisco, and colleagues found it took CJD patients almost eight months from the onset of symptoms until they were correctly diagnosed, on average. By then, most only had a few months left to live.
The study included 97 people, age 26 to 83, referred to the UCSF Memory and Aging Center between 2001 and 2007. Seventeen of them had been correctly diagnosed with CJD at their first assessment - but it took much longer for others.
Caselli said that because symptoms of different brain-related disorders often overlap, magnetic resonance imaging, or MRI, is typically the best way to see what's going on in the brain and make a diagnosis. But even so, that doesn't always work.
"The challenge comes when the MRI findings are not as definitive as you'd like them to be," he told Reuters Health.
Some other invasive tests, such as spinal taps, can differentiate meningitis from CJD.
Geschwind said he often tests for infections such as syphilis and HIV in people with early symptoms similar to CJD. Dementia that comes on quickly can also be caused by cancer antibodies that attack the nervous system, he said.
A small fraction of CJD cases are passed down in families and the disease can be transmitted through infected brain tissue. But most cases occur when normal proteins in the brain spontaneously become infectious, for no clear reason.
"Any time there's a patient with a rapidly progressing dementia, I mean going from completely normal function to not being able to function in a few months to less than a year, I think CJD should be (considered)," Geschwind told Reuters Health.
"But so should many of these other conditions, some of which are treatable."
"It still is true that one should not accept the diagnosis without being sure that one has had a thorough evaluation," Caselli said.
"The more definitive one can be, the more one can save the patient who's pretty oblivious through all this from getting dragged from one doctor to another," he added.
"Once the diagnosis is credibly established, it really probably is best to start thinking about palliative care hospice, the comfort of the patient and the family so that people can start to deal with this diagnosis and the time they have left."

Bayer wins early U.S. approval for colon cancer drug
(Reuters) - The U.S. Food and Drug Administration on Thursday approved a new drug for advanced colon cancer developed by Bayer AG a month ahead of the agency's expected action date for a decision on the medicine.
The drug, to be sold under the brand name Stivarga, was approved to treat colon cancer that has progressed after prior treatment or has spread to other parts of the body, the agency said.
In pivotal clinical trials, Stivarga -- known chemically as regorafenib -- helped patients live on average 1.4 months longer than those who received standard treatment without the Bayer drug.

Spike in heart failures follows Japan's 2011 quake

NEW YORK (Reuters Health) - Following the massive earthquake and tsunami of March, 2011, which devastated parts of eastern Japan, the number of heart failure cases spiked in Miyagi Prefecture and remained elevated for six weeks, according to a new study.
The number of people diagnosed with cardiac arrest and other heart problems also jumped immediately following the earthquake, but tapered back to earlier levels within three weeks.
Dr. Hiroaki Shimokawa, one of the authors of the study and a professor at Tohoku University Graduate School of Medicine, told Reuters Health that factors such as drug discontinuation, cold temperatures in the short term, excessive salt intake from preserved food, and mental and physical stresses over a long period likely contributed to the sustained increase in heart failure cases.
The quake, in Japan often called the Great Earthquake of East Japan, registered at a formidable 9.0 on the Richter scale, and resulted in more than 15,000 deaths.
Shimokawa and his colleagues wrote in the European Heart Journal that Miyagi Prefecture was the closest to the epicenter, and more than 9,500 people died there.
In an email, the researcher said that earlier studies have linked earthquakes to short-term increases in cardiovascular problems.
And a previous study found that months following the 2011 earthquake, workers at nuclear power plants in the region suffered from high rates of stress and depression. (see Reuters Health report of August 14, 2012).
Shimokawa and his colleagues wanted to look at longer-term effects on heart health, and they gathered the medical records of all people brought by ambulance to one of Miyagi Prefecture's 57 hospitals in the month preceding the earthquake and the four months following.
They compared the number of stroke, heart failure, cardiac arrest and pneumonia diagnoses during this time period in 2011 to the same time period in 2008, 2009 and 2010.
The number of people diagnosed with cardiac arrest jumped immediately on the day of the earthquake. For instance, there were 17 cases on March 11, 2011, compared to five the day before and between two and four on March 11 in each of the three years prior.
The reason for the swift jump in cardiac arrest cases appears to be that the heart is "sensitive to sudden onset of stresses," said Shimokawa.
Heart failure numbers, on the other hand, remained normal for several days following the earthquake, but then increased by March 15.
The cardiac arrest rate returned to levels seen in previous years two weeks after the earthquake, whereas the heart failure rate remained elevated.
The study could not pinpoint the causes of the increase in heart problems.
"I'm not really sure whether indeed it's the increase of cardiovascular disease by stress (from the earthquake) that's counted here, or whether it's simply the consequence of the catastrophe they've been in - that is, the trauma, the breakdown of lifelines, water, food, etc.," said Dr. Gerhard Steinbeck, a professor at the University of Munich in Germany, who was not involved in the Japanese study.
In an editorial, Steinbeck and a colleague wrote that there could be some inaccuracies in the ways heart failure and cardiac arrest were counted.
For one, the definition of heart failure and how it was diagnosed was not given. Steinbeck said making such a diagnosis is time-consuming and difficult, especially in the midst of a catastrophe.
Additionally, the causes of cardiac arrest are not detailed, and they could reflect cases of people who drowned.
Therefore, he said, it's not clear whether the numbers reflect a prolonged exacerbation of heart disease caused by the earthquake itself or the negative health effects caused by the aftermath of the disaster.
"Whatever the mechanisms... the data clearly show that it's a real challenge to any healthcare system," Steinbeck told Reuters Health.
Steinbeck said the study is useful in helping healthcare systems prepare for the extended needs after natural disasters.
"It's not a matter of hours or days thereafter, but any healthcare system has to deal with increased emergencies and cardiovascular disease not for days, but months," he added.

NEW YORK (Reuters) - When Dr. Marty Makary was a medical student, staffers at the Boston hospital where he was training had a nickname for one of its most popular surgeons: Dr. Hodad.
"Hodad" is an acronym for "hands of death and destruction": Despite his Ivy League credentials and board certification, the surgeon had an unfortunate tendency to botch operations so badly that patients often suffered life-threatening complications.
But he was also one of the surgeons most requested by patients, including celebrities, thanks to his charming bedside manner and their lack of understanding about what caused their post-op problems.
Makary, 42, aims to end the professional code of silence that allows colleagues like Dr. Hodad to thrive. Now a cancer surgeon at Johns Hopkins Hospital in Baltimore, Makary has just published the book "Unaccountable: What Hospitals Won't Tell You and How Transparency Can Revolutionize Health Care."
It outlines the extent to which doctors and hospitals suppress objective data about how patients fare in their hands and argues for clear, publicly accessible statistics to help people make the best choices when it comes to treatment. Hospitals and physicians, he argues, should collect "outcomes data" on everything from how many knee-replacement patients walk without a limp to how many prostatectomy patients become incontinent.
Without that, "patients are walking in blind" every time they choose a hospital, Makary said in an interview. With rare exception they have no way of knowing whether they will receive appropriate care or be one of the 100,000 patients killed or 9 million harmed every year in the United States because of medical mistakes.
"There is terrible guilt about keeping quiet, but there are strong social forces against speaking up when you think something doesn't look right: It can get you fired," said Makary. (HealthGrades, a Denver company that develops and markets quality and safety ratings of healthcare providers, rates Makary a "recognized doctor" based on his training and record of no disciplinary actions or malpractice claims.) "You realize as a young doctor that you've walked into an industry with a very dark side."
CLEAR AS MUD
In no U.S. state can patients find out what a surgeon's rate of complications is, how many mistakes a hospital makes, how many avoidable deaths it has or almost anything else about a provider's record of care.
Most ratings, from magazines to websites, reflect softer metrics. In the closely watched hospital rankings issued by U.S. News & World report, "reputation," or what specialists think of a hospital, counts 32.5 percent toward overall scores. Patient volume, number of nurses, use of advanced technologies and 30-day mortality rates also count.
The federal government collects and makes public some measures, such as hospitals' rates of complications and mortality after certain procedures, on the Hospital Compare website. About half the states require hospitals to make public what percentage of patients develop infections. While that's better than nothing, says Dr. John Santa of Consumers Union, publisher of Consumer Reports, providers have largely succeeded in hiding their records.
"Despite the best efforts, if hospitals don't have to report something they don't," said Santa.
For example, a regular survey by Johns Hopkins asks staffers at 60 hospitals about safety and teamwork. Studies show that hospitals scoring high on the surveys have fewer surgical complications and better patient outcomes. But hospitals participate "under the condition that the results remain top secret," said Makary.
Specialist groups also gather data, including the Society of Thoracic Surgeons, which tracks national heart-surgery outcomes. Only one-third of hospitals have agreed to post their results on the society's website.
Santa believes patients should have far more data on outcomes, such as what fraction of hip-replacement patients develop infections and what fraction of heart-bypass patients survive, not just currently available information on whether providers follow medical guidelines.
The reason? Good practices may not be a reliable proxy for good safety. A hospital's rate of providing antibiotics after surgery, for instance, does not always correlate with patients' infection rate, said Santa.
The Joint Commission, an independent non-profit that certifies and accredits hospitals and other providers, last week released its annual report summarizing how well 3,300 hospitals did on measures of quality and safety.
Patients can see that a particular hospital was a "top performer" in pneumonia care, meeting criteria such as taking blood cultures in the intensive care unit. But unless a hospital was specifically cited for exceptional care, patients have no way of knowing how good or bad relative to others it is.
More outcomes measures - whether that knee replacement patient walks again, or even dies on the operating table - will be made public in coming years, said Nancy Foster, vice president for quality and patient safety policy at the American Hospital Association. Patients typically choose a hospital based on what their doctor or friends recommend, she notes, adding, "I think there is a lot of opportunity to enrich that process with hard data. The measures currently available are not as consumer-friendly as most of us would like."
THE EXCEPTIONS
Makary notes several models of transparency that have shown promise. New York, Oregon and California require hospitals to report death rates from heart bypass surgery, adjusted for how sick patients were and other factors to make the comparisons fair.
Transparency has benefited patients. After New York made its data public in 1989, hospitals scrambled to improve, and death rates from heart surgery fell 41 percent in four years.
Vitals.com, a doctor-reviews site launched in 2008, recently began incorporating outcomes for cancer and orthopedic surgery from a number of large hospitals into its ratings, said chief executive and co-founder Mitch Rothschild.
"Individual facilities recognize that if they don't weed out bad practitioners, they'll get creamed as Medicare starts penalizing hospitals for poor performance, so they collect these metrics and share them with us," he said.
For other outcome data that hospitals chose not to share, Vitals filed a Freedom of Information Act request to access the government's Medicare health program for the elderly.
"After a year and a half, as legal fees mounted, we gave up," Rothschild said. The government maintains the data cannot be made public for reasons of privacy and others.
In the meantime, the pitfalls for patients are many. When Makary looked Dr. Hodad up years later, he was still thriving and had a five-star rating on a popular review website.
Makary regrets keeping quiet during a residency at a university-affiliated community hospital that boasted of its "comprehensive breast cancer center" and "No. 1 ranking."
Both statements were inventions of the hospital's marketing department, which can make all sorts of claims as long as they are vague enough not to fall afoul of truth-in-advertising laws. The assertion that patients "may" or "often" do better at a particular hospital is allowed, for instance, as are subjective terms like "comprehensive."
Based on such claims, a young patient Makary calls "Gretchen" who needed breast-cancer surgery believed she would get superb care.
In reality, the small hospital did only a few dozen such surgeries per year compared with hundreds at major hospitals. It did not have the expertise to do breast-conserving and -reconstruction surgery, nor were its surgeons adept at the latest procedures.
Makary said he was bothered at the time by the hospital's disingenuous claims and worried for Gretchen, though he did not warn her. He did ask if she'd considered other hospitals, but even that placed him "on thin ice with my own job."
The operation was horribly botched, leaving Gretchen deformed. Not knowing any other outcome was possible, Makary said, she considered herself "very blessed" just for being alive.

Several ill in Canada after eating possibly contaminated steaks
(Reuters) - Several people became ill after eating steaks that may contain E. coli bacteria sold at a store in Edmonton, Alberta, the Canadian Food Inspection Agency said on Wednesday.
The announcement follows a series of beef product recalls that have raised concerns about meat processing standards in Canada.
The CFIA warned the public not to eat Kirkland brand beef steaks sold between September 4 and 7 at the Costco Wholesale Corp store on 50th Street in Edmonton.
The agency has not identified the supplier of the potentially contaminated beef, said CFIA spokesman Tim O'Connor.
One of the suppliers to the store is XL Foods' Brooks, Alberta, plant. Beginning in early September, privately held XL Foods has voluntarily recalled more than 250 beef products made at the plant after positive findings of E. coli.
E. coli bacteria can cause serious and potentially life-threatening illnesses.
XL Foods issued a statement saying it is "deeply concerned" for the people who became ill, adding that there has been no definitive link between its products and the cases of illness.
The United States halted imports of beef products from the XL Foods plant in Brooks, which is one of the largest in Canada, on September 13.
Canada is the sixth-largest beef and veal exporter in the world.
The CFIA has completed its review of the plant's food safety controls and found XL Foods could not demonstrate that it regularly updated its plan to control E. coli in the facility, the agency said.

Ministers maintain the NHS is performing well despite the pressures
Fresh fears are being raised in England that cuts will have to be made to the front line of the NHS if it is to cope.
The government has promised to protect the health service, but research by the King's Fund and a poll commissioned by the BBC both raise doubts about that.
The BBC survey of 1,005 people showed three in five believed services would be hit in the current climate.
Meanwhile, a King's Fund report, based on interviews with finance chiefs, said 2013 could mark the turning point.
The majority of the 45 NHS directors of finance who took part in the think tank's study said they were currently managing to make savings without harming care.
The King's Fund said these sentiments were supported by the latest performance statistics which showed the NHS was performing well.
Waiting times in A&E and for non-emergency operations, such as knee and hip replacements, had fallen slightly and were well within target, while hospital infections rates continued to drop.
'Strain on services'But 2013 was seen as the year when it could start to unravel by many of the finance directors.
A total of 27 of the 45 managers who took part in the online questionnaire said there was now a high or very high risk that NHS would not meet its savings targets of £20bn by 2015.
Nineteen of the panel said they expected care to worsen over the next few years, with only eight believing it would get better.
The BBC poll, carried out by ComRes, asked members of the public in England a series of questions about the NHS.
Some 61% agreed that they expected the NHS would have to stop providing some treatments and services in the future due to rising costs and increasing demands.
Nearly three-quarters also said they did not trust the government with the health service.
Interestingly, over half of respondents said it did not matter whether private firms provided care as long as it was free of charge - the government's reforms have come under heavy criticism amid a perception they would lead to greater private sector involvement.
NHS 'on track'Prof John Appleby, chief economist at the King's Fund, said: "There are signs that future years will be harder.
"The end of the public sector pay freeze next April may add to financial pressure and increase the strain on services.
"The difficulty will be finding ways to absorb these costs without compromising the quality of care for patients."
But health minister Lord Howe maintained the NHS was "on track" to achieve its savings target.
He said £5.8bn was saved last year, while performance remained good.
"Waiting times have been kept low, infections have been reduced, there are more doctors, more diagnostic tests and more planned operations," he added.

High sleeping pill use 'worrying'

A third of people are affected by insomnia in the UK
Too many people may be damaging their health by self-medicating with sleeping pills, according to the Royal Pharmaceutical Society.
It said half of people with insomnia diagnosed themselves and took medication without seeking medical advice.
However, the society said insomnia was often part of other physical or mental health problems which needed treating.
The warning was based on the findings of a survey of 2,077 people.
Insomnia is difficulty in getting to sleep, staying asleep or getting enough good quality sleep night after night. One in three people in the UK are thought to have bouts of insomnia.
It can be caused by psychiatric problems such as depression, anxiety disorders and schizophrenia. Other illnesses including heart disease, Alzheimer's disease and hormonal problems can also disturb the normal pattern of sleep.
In the survey, 30% of people said they had taken sleeping pills for more than a month without getting advice while 14% had gone six months.
One pharmacist, Paul Johnson, said: "It's worrying that so many people are overusing sleeping remedies.
"They can be effective for short-term treatment of mild insomnia but should not be taken for long periods without advice because they can hide a serious health problem which could get worse if it remains untreated.
"It's never a good idea to take any medicine long-term as a result of self-diagnosis, as you can end up treating a symptom rather than addressing the root cause of your problem."
